I saw that show and Martha Stewart was clearly in on the joke – why else would they have them make a bed of all things?! It was so set up but it was funny.
Here is the thing, Baron-Cohen is what Andy Kaufman was in the late seventies to the early eighties, the in character shock comedian but Baron is adding in gross props and taking the leud behavior to a ridiculous extreme. I am sure that he told Stewart and Leno what he what he was going to do a head of time. The trick is to make the joke as real as possible so that the audience does not suspect that it is only a gag.
I was actually quite impressed with Martha Stewart for being a good sport about the whole thing. Nobody can resist Borat’s charms.
